How to fill out order form & ordering details
Fill out all of the information on the fax order form completely otherwise we will be unable to fulfill your order. We will contact you to confirm your order via e-mail or telephone. If we don’t contact you to confirm, then we did not receive your fax & please fax it again.

Shirts are $20 including sales tax. Shipping is $5.65 for one shirt and $11.40 for two or more shirts within the United States. For international orders, we will let you know what the shipping costs will be dependent on the country. When we confirm your order via e-mail or telephone & we will confirm shipment & the total cost of your order.

Orders are only processed Monday thru Friday. You may fax at any time, but we will only confirm orders 1-5pm on Monday thru Friday.
